Indicates changes in a candidate's most current plan of study.


• Approval of Transfer Credit Form for Graduate Degrees
Use 1) upon admission as approval of transfer credits (up to 6 credits), 2) after admission to pre-approve transfer work to satisfy degree plan requirements. May need to accompany Request for Changes in Plan of Study.


• Program Transfer Form

Use to request candidate materials be considered for admission by another program or department. Note: additional materials may be necessary for receiving program to adequately consider candidate qualifications
