Family Support 360 Transition Project

Family Support 360, a project of the Paul V. Sherlock Center on Disabilities, engages and supports families of youth with significant developmental disabilites in a comprehensive planning process to develop a smooth transition from high school to meaningful adult life.

The Family Support 360 Transition project is funded by a grant from the Administration on Developmental Disabilities, U.S. Department of Health and Human Services.

How do we support families?

Who can become a part of the Family Support 360 Transtion Project?

Families with students, ages 14-21, who have significant developmental disabilities; and are likely to be eligible for services through the RI Developmental Disabilities after graduation.

Who can refer families to the project?
