Assistive Technology
Family Support 360, a project of the Paul V. Sherlock Center on Disabilities, engages and supports families of youth with significant developmental disabilites in a comprehensive planning process to develop a smooth transition from high school to meaningful adult life.
The Family Support 360 Transition project is funded by a grant from the
Administration on Developmental Disabilities,
U.S. Department of Health and Human Services.

How do we support families?
- Sherlock Center Family Support Coordinators meet with families and students to provide key information
about transition planning and their roles in the process.
- Project staff facilitate a person-centered plan (or MAP) 57 kb (PDF)
- Staff work with families and their IEP transition teams to design quality educational, vocational, and community experiences to help students achieve
their goals.
- The project assists families in choosing adult providers, including options for self-directed supports.
- Family Support Coordinators help families through the application process for the Office of
Rehabilitation Services (ORS) and Division of Developmental Disabilites (DDD).
- Staff provide families with information on topics including guardianship, financial planning, system advocacy, due process, housing, transportation options and
community particpation
- The project also provides opportunities to connect with other families in transition, and access to other
resources for family support.
Who can become a part of the Family Support 360 Transtion Project?
Families with students, ages 14-21, who have significant developmental disabilities;
and are likely to be eligible for services through the RI Developmental Disabilities
after graduation.
Who can refer families to the project?
- Family Members
- Special Educators
- Agencies involved in the transition planning process
